Case-History Form
For the first appointment, please arrive 10 minutes earlier to give yourself time to fill in the case history form. Alternatively please download and complete the form on the page below and bring it in at the time of the appointment.
For young children with behavioural, developmental issues please make sure that you bring someone to accompany the child in the waiting area as we discuss the child’s situation. The child can then come in for the Homeopathic case taking. Alternatively, Please book separate appointments to discuss the child’s situation. Homeopathic remedies can only be prescribed after observing the child.
